At WeKare Disability Support Services (WDSS), we use a person-centred approach that focuses on delivering the best of care to participants across a broad range of NDIS core support and capacity-building categories. Our carers offer a friendly and supportive presence,  always respecting your beliefs and values. Your WDSS care team places you at the centre of all decision-making, because no one knows your own needs and goals better than you do.

 

Our comprehensive NDIS core supports and capacity building services are offered throughout Victoria.


Support Co-ordination

Our Support Co-ordinators are highly-credentialed, with qualifications in Social Work and a background in helping those with a disability and their families. We work with all participants - and especially those requiring intensive support - to get ultimate value out of their NDIS plan. Our aim always is to help you maximise your choices, build your own capacity to manage supports and ensure NDIS services support your long-term goals. Our NDIS support co-ordination is provided under the following NDIS registration groups:

  • Support Connection - We assist you to understand all aspects of your plan, and connect you to providers and the broader systems of supports to help you achieve your goals.
  • Co-ordination of Supports - We work together to design and build your supports and help you build and maintain your networks.
  • Specialist Support Co-ordination - If you have complex needs or are in a crisis situation, we focus on outcomes and help to reduce complexity in your supports.

 

Community Participation

WeKare Disability Support Services NDIS Community Participation

We passionately believe that people of all abilities should have an opportunity to participate in a full range of community activities. We work with you to identify your skills, interests and talents and match those with activities happening in your local area. By getting out and about in your community, meeting new people and trying new things, you can not only live a more rewarding life, but move further towards reaching your goals. We support you to take part in all the things that matter to you.

In-Home Support

WeKare Disability Support Services NDIS in-home support assist personal tasks household tasks

We offer individualised services to children and adults, specifically tailored to help you live independently, either in your own home or in a group living arrangement. We not only help with all the tasks that need to be completed around your home, but provide respectful and dignified personal care. We also help you build your own skills over time, to live as independently as possible.

What kind of NDIS in-home support can we provide?

  • Household cleaning, laundry, washing and ironing
  • Personal grooming, bathing and toileting
  • Cooking and meal preparation
  • Light garden maintenance
  • Helping you get around the house
  • Accompanying you to appointments
  • Shopping and other errands

Our NDIS in-home care can help families or carers to have time away from care giving to refresh and renew, whilst also helping participants to improve their quality of life by remaining independent.

 

Accommodation & Tenancy

WDSS NDIS services accommodation and tenancy support

WDSS can help you find a place to live that suits your lifestyle, abilities and preferences. We're with you every step of the way, helping to manage applications and agreements to liaising with all the parties involved in getting you into the place you call home.

What accommodation and tenancy tasks can WDSS assist with?

  • Helping to identify and locate housing that suits your abilities and preferences
  • Liaising with landlords regarding tenancy agreements
  • Helping you to retain the accommodation you already have
  • Help applying for rental properties
  • Help maintaining the premises and adhering to lease agreements

 

Assisted Holidays

We are passionate about ensuring that people of all abilities can travel and participate in holiday memories to last a lifetime. That's why we provide 1:1 support with assisted holidays, whether it's local day trips and short stays or interstate travel to really see what Australia has to offer. We ensure every holiday is truly meaningful and memorable.

Our carers have relevant police and working with children checks and hold current first aid certificates. All are dedicated to ensuring your holiday is as fun and rewarding as you want it to be. Our supported holiday options are tailored to cater to people with mild to moderate learning disabilities who require a low level of personal care, and have minimal mobility challenges.

We carefully listen to the kind of holiday you're looking for and then work with you to customise a trip that suits your requirements and budget. We tailor our support to your needs, ranging from small groups with a minimum of 1:3 support and 1:1 support depending upon your requirements.

 

Skills Development and Training

We help you build confidence, self-esteem and independence, so you can live as autonomously as possible and participate in all life has to offer. Training by our friendly and highly-experienced team can be provided either individually or through group activities and can include skills in daily life activities, communication and social skills, problem solving and managing funding of supports.

By participating in these activities, you improve your ability to manage the challenges of life, including building your networks of support. And if you have a particular ability, skill or unique interest, we work with you to put a program of learning together that helps you achieve your goals.

Our assistance with NDIS skills development and training includes:

  • Travel training, including the use of public transport
  • Cooking, menu planning and nutrition skills
  • Managing money, budgeting and bill payment
  • Computer skills
  • Personal safety training
  • Supportive training for parents and carers

 

Short-Term Accommodation

WeKare provides Short Term Accommodation (STA), including respite. Short-Term Accommodation is designed to give both you and your support network a hard-earned break. It is funding covered by NDIS for support and accommodation away from your usual home, for up to 14 days at time. It’s often accessible when your regular care team isn't available, or to give you the chance to try new activities in fresh surroundings.

STA includes all expenses in a 24 hour period including assistance with daily personal activities, accommodation, food and other activities. It gives you a chance to try new things, make new friends or develop new skills. At WeKare, our goal is to make our short stay accommodation your home away from home.

 

Medium-Term Accommodation

WeKare provides Medium Term Accommodation (MTA) so you have somewhere to live while waiting for your long term housing solution to be confirmed. MTA might be used in situations such as bridging accommodation for younger participants at risk of being admitted to aged care, those who are waiting for home modifications to be finalised or participants ready to leave hospital who are waiting for disability related home modifications to be completed or for permanent accommodation to become available.

NDIS Medium Term Accommodation covers up to 90 days of funding in furnished accommodation as well as core supports.

 

Supported Independent Living

Supported Independent Living is NDIS funding that helps people with disability to live as independently as possible. SIL funding is typically for people who need 24/7 support while living in a shared home. It exists to cover the cost of support staff in the house. Whereas Supported Independent Living is the funding for support services, Specialist Disability Accommodation is the accommodation in which those services are delivered.

WeKare's SIL services can help you enjoy your independence, and build your confidence while living as independently as possible, with all the support you need.

 

TAC Case Management

WeKare provides reasonable, strengths-based case management services for a client who has sustained injuries resulting from a transport accident. A TAC (Transport Accident Commission) claim is an insurance claim you make if you are injured in a transport accident.
